Meridian Home Health, a Specialized Home Health Agency, equal opportunity employer, seeks exceptional, qualified Physical Therapist to service its Skilled Intermittent Home Health patients in Bay Area and the surrounding communities.
We are seeking a personable, compassionate, and dedicated Full-Time Home Health Physical Therapist who is committed to providing exceptional, patient-centered care. The ideal candidate will build meaningful relationships with patients, serve as a trusted partner throughout their rehabilitation journey, and help them restore mobility, regain independence, and achieve their highest level of physical function and quality of life.
Job Description:
- Provide physical therapy services to patients according to written physician's plan of care.
- Initiate physical therapy program and instructs other personnel/ or family/ caregiver members in certain phases of physical therapy.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Supervises Physical Therapy Assistants according to organization policy and state regulations
- Prepares and submits a clinical progress summary based on the attainment of goals
- Participates in discharge planning for patient
- Provide accurate and timely documentation
- Participates in coordination of home health services with team
License/Certification:
- Current PT License
- Current BLS/CPR Certification
- Current CA Driver's License
Qualifications:
- Excellent communication and interpersonal skills
- The ability to create a positive, nurturing environment
- The ability to work well and make decisions in a high-stress environment
- Quick submission of timely and correct documentation
